Files to exclude from vscode, listed in .vscode/settings.json are not excluded in the focused folder.
The specific case I observed are .meta file created by Unity (and added to vscode exlude) still being visible in the focused folder. Unity makes one .meta file per file and folder in it's project folders. This means the focused folder has twice as many visible items than expected.
