Publish spec as First Public Working Draft #111

@draggett, the spec is sitting in a special branch on GitHub.

To get it follow these instructions:

git clone -b First-Public-Working-Draft git@github.com:sysapps/telephony.git
open telephony/FPWD.html

Please note that the file links to the following images:

If you need to modify the document, then you can either modify the source (index.html) or the FPWD.html file (up to you).

Please make sure that blue banner remains in the document.

Merged in some small fixes from the last week in to the document. They are summarized here in #115.
