pvh: initial patch of pvh port to mini-os for clickos #1

This patch is rebased to mini-os for clickos by using the commit of 1c32dd9f7a3ed in https://github.com/hwanju/xen (minios-pvh branch). This initial patch doesn't handle new features of mini-os for clickos like suspend/resume.

Looks like Github doesn't notify when the merge is going to a branch other than the one specified in the PR.

Yeap, cool! btw, I added a missing file xen-features.h in this branch "pvh" 1e4d561401b555bb649fb5eea92ec654dbf210f1 . This change has slipped me through the review, but it is fixed now.
