Support for Smarty > v2 #199

When will donrec support the most recent smarty verion available in civiCRM?

I assume this requires a civix upgrade, which has been merged for the next minor version (2.3) in #193. This, however, drops support for CiviCRM < 5.60. I think that's ok for backporting this PR to a 2.2 version.
