Tokens missing / inaccessible #4

There is still various issues with the tokens in this extension:

  1. contact custom fields are missing
  2. the custom_group_name.custom_field_name causes issues in smarty

This should now work for all custom fields using both, "real" tokens, and the variables additionally passed into the template with the adapted notation of custom_group_name__custom_field_name.

This did not yet work for the "human-readable" Smarty variables for event custom fields, because the . separator was not being replaced with __, which 8685a01574df0fd8d3eee59ba2735a3b6cb32191 fixes.
