Alt+Numpad codes shouldn't depend on NumLock #112

Currently Alt+NumPad codes are only accepted when NumLock is on. Yet
generally in Windows, they do not depend on NumLock. Also, Alt+NumPad codes
should be disabled when Application Keypad Mode is on (with DECKPAM on and
DECCKM off).
