Maybe this is an issue with 0.54 and no longer relevant, or perhaps I just don't understand how to use the keyword, but as faras I can see it shows now effect. If I'm mistaken what is the combination that works?

I introduced kana_head to v0.57 at 2021-06-06. https://github.com/t-tk/upmendex-package/blob/master/source/ChangeLog

Samples of input are https://github.com/t-tk/upmendex-package/blob/master/doc/samples/alphabet/mdxsty_ja31.ist https://github.com/t-tk/upmendex-package/blob/master/doc/samples/alphabet/mdxsty_ja32.ist https://github.com/t-tk/upmendex-package/blob/master/doc/samples/alphabet/mdxsty_ja33.ist

Outputs are https://github.com/t-tk/upmendex-package/blob/master/doc/samples/alphabet/output/alpha_ja31.ind https://github.com/t-tk/upmendex-package/blob/master/doc/samples/alphabet/output/alpha_ja32.ind https://github.com/t-tk/upmendex-package/blob/master/doc/samples/alphabet/output/alpha_ja33.ind

ah, then the problem really seems to be that the documentation found in TeXLive does not match the program found in TeXlive, ie, the program is at 0.54, but the man page etc are already documenting 0.57 or perhaps even later. Is there a chance that the two are kept in sync somehow?

In the future release, I will submit documents for a new version to CTAN after code freeze of binaries in TeX Live subversion repository (usually in March). I expect that the difference of versions between documents and binaries will be reduced.

I submitted package of v1.07 to CTAN today. I requested to Karl-san that it will be installed only in TeX Live 2023 pretest & release version, but NOT in 2022 regular tlnet repository. In the future release, the manual and binary will be always consistent.
