Implement a feedback mechanism #25

User feedback at the early stages of our project is important. As such a few good questionnaires would need to be created for use in conjunction with the app.

Where is the domain hosted, on what kind of package? Hosting/VPS/...? I guess we can simply get a php feedback form, I will look at my websites for a ready one.

At the moment we're on Amazon EC2. I've added you to the developer gdocs.
