Adds instructions, closes #139 #140

This worked for me. Adding it to .profile or .zsh_profile simply did not, even starting from a login shell.

Over in the App-Rakubrew repo the notes on how to install are now a lot more fine grained and specific for the shell the user is currently running. I think this PR thus doesn't apply any more. So I'm closing this. If I'm wrong, please reopen!
