Make the incoming call window appear on top of all other windows, for easy answer / decline. #425

It's annoying I have to click the growl notification, and then click "Accept".

People don't have much time to answer a call before it goes to voicemail, so 
making it streamlined is important.

Also, it's annoying to have something ringing in the background and to have a 
series of steps (switch to telephone application, locate and click button) to 
get it to stop.

Bonus: use green and red buttons like skype (and everything else) so that I 
don't have to think about which one to click.
