If You used Titanium backup to restore any data other than games.
re install clean and do not use Titanium. If the problem still exists
Than please file the report.

What Phone Are you using--Maguro(GSM)-TORO(Verizon)-TOROPLUS(Sprint)
(Only issues from 4.2.1 and beyond are currently being accepted)
ROM Version 4.0 and higher

Did you install after wiping data/cache/davlik cache or dirty flash(no
wipe)  This must be answered honestly for us to help.

Just suggestion:
-could you add option to enable repeated sound notification (kind an attention 
grabber) in addition to led notification? Useful to give alert when phone flip 
down or inside pouch. User can choose sound frequency (repeat every minute, 
etc) and duration until it reset itself. Screen no need to switch on when sound 
active to save battery.
