Merge relayer and grafana docker, add setup documentation

Summary of changes

• Merge docker/prod and docker/grafana into single folder and single docker-compose.yml
• Update Docker instructions in readme
• Dont expose ports publicly in docker-compose.yml

I dont have a server to test this with but it should work in theory. It also doesnt include any nginx config for Grafana (again, would need testing).
