insertimage bug on ie #143

What steps will reproduce the problem?
1. open the page with ie (don't focus on the textarea)
2. click insertimage, input a url or select a file
3. click ok

What is the expected output? What do you see instead?
The image will insert to the top of page, outside of the textarea

What version of the product are you using? On what operating system?
all version, win xp

Please provide any additional information below.
