Overview

For device form, get duplicate code from base auth template.

mimics form changes in #43

Changes

changed to extend base-auth.html, not base.html
removed all code that is in base-auth.html

Testing

Verify device-code.html form is still functional.

UI

Note I cannot test, because containers are unable to be run on my system (ARM architecture), and I have not requested a VM for TAPIS development.
