Simplify api #45

Add ignoredPackages option, populate internals by default.
Remove bound singleton, export StackUtils class only

Fixes #15

@novemberborn From what I can tell this will be non-breaking for ava, will this serve your needs? In particular please take a look at the new RegExp that is being generated for the ignoredPackages, let me know if you think any changes are required for ava to use this new option.

@coreyfarrell I'm not really up to speed on what this package does and how we use it. I'm sure we'll manage.
