Add the "unnumbered" appendices #48

(But I gave them numbers to keep things in order.)

(I left out "8. Name Index" because all the references are via page number, which isn't very useful in this format. But I could transcribe it if you want it for completeness.)

So does the presence of mkdocs.yml mean that this manual should appears on a docs site somewhere? I looked at readthedocs.org, didn't find it.

It probably should be. I'm setting it up on readthedocs.org right now.
