Doc cleanup #31

I just tried to set up accounts with gmail and with outlook.com. The names of things are subtly different now than they were in the README, so i think this is an improvement (though it's still quite far from a simple or automated process)

I note that when fiddling with Outlook's configuration web interface, i was unable to "add API permissions for SMTP.Send"
