Remove officer nomination anonymity and allow nominator to provide a few sentances

The officer corps has voted to change the online nomination process to include the name of the nominator in the notification email that the system sends to the nominated individual. An additional request would be to add a text box to the nomination page and insert the contents of the text box into the email body as "comments from the nominator."
