Editorial: Various editorial updates #166

Applies similar updates as https://github.com/tc39/ecma402/pull/822 and https://github.com/tc39/ecma402/pull/827, including:

Updates ecmarkup.
Ensures every operation has a complete structured header.
Mathematical and Number values are correctly used.
Consistent wording when appending entries to lists.
Correct field access for [[<name>]] form.

And fixes the unresolved editor notes for PartitionDurationFormatPattern.

And finally:

Fixes #151
Fixes #154

@ben-allen @ryzokuken We should review and merge this before the other smaller open PRs since this is going to probably start getting merge conflicts.
