Cleanup the spec and the polyfill

I expect no observable change in the runtime. If anyone finds this PR is changing the runtime behavior, please tell me!

• Remove the lastValue internal slot since it is unused.
• Remove the unused loop (it was used when it was an iterator in the spec.)
• Prefer not to mutate the variables to make it easier to read.
