Repointer the docs to tds-fdw/tds_fdw. #126

The docs still refer to https://github.com/GeoffMontee/tds_fdw. It appears this repo has since been moved.

If merged, the docs now say https://github.com/tds-fdw/tds_fdw thus eliminating one potential source of confusion.

Can one of the admins verify this patch?

Thanks, @StevenBlack! This has been merged. GeoffMontee/tds_fdw is forwarded to tds-fdw/tds_fdw, so the old links still worked, but it's good to be consistent.
