Remove 9.2 and 9.3 (EoL)
Add PostgreSQL 11

To be merged after https://github.com/tds-fdw/ci-build/pull/9

Fixes #188

@GeoffMontee, please review, but do not merge, as I will need to generate the images after https://github.com/tds-fdw/ci-setup/pull/2 is approved and merged. I will merge the PR after that.

I am not sure why ubuntu14.04 is failing for PostgreSQL. Not directly related to this PR I'd say, but I am reviewing it.

Fixed. The problem is that I did not have the last changes on master on my fork, before creating the branch (and one of the changes was improving PostgreSQL detection so we could work with Ubuntu).
