fix trailing slash problem #129

This is to fix issue #128 . I think the only relevant places are in the four documents and the header template used to generate the term list document.

@nielsklazenga, if you can give this a look, merge it if it looks OK.

Hey @nielsklazenga @rebeccasnyder @ramorrismorris, can one of you look this over and merge it? I think this should be a straighforward fix, but as a general policy, somebody besides me should do the merge as a cross-check on my work.
