Format image doc #193

Hi @magpiedin . Take a look at the formatting that I added to your doc. I didn't really change the content significantly -- just added a standardized header, the standard TDWG license at the bottom, and some section numbers.

I also did some funky stuff (delete then re-add) to force it to change the file name to all lower case. I'd like to keep that pattern as much as possible so that the URLs are consistent.

The introduction section and abstract are a bit sparse, so we might want to flesh them out more. See https://github.com/tdwg/ac/blob/format_image_doc/image/examples.md for the rendered page.

Also, I'm not really sure that we want to do any kind of official versioning on this doc, so maybe we can just put the last-edit date in the header rather than the creation and most recent issue date.
