Material entity term changes update

Includes:

• updated list of terms doc with templated header and links for contributors
• update Quick Reference Guide
• generate new versions of all guides except RDF using templated headers and contributor links, other minor update edits
• revisions to build scripts
• add script to move current standards documents pages to versioned pages with revised dates
• correct various small metadata errors, facilitate correct placement of record level terms in list of terms index
• housekeeping changes in repo
