Allow (mostly) arbitrary form definitions

• Change DSL to allow any element names and options
• Move small number of options (those which apply apply to any element) into a base element class (see “allow conditional elements” as example of this)
• Create standard AST output for non-custom elements
• Remove any whole element classes where there is no custom behaviour
• Review which elements (if any!) need to remain as custom classes
