Happy Elementor Addons Pro Ver. 1.17.1. Using Elementor Pro 3.2.2

There are no fillable fields showing on any of the Block (Contact Pages) offered by Happy Elementor Addons Pro. Only basic information appears for each page. Example: Name, email address do not show on any available blocks offered.

I tried disabling plugins, and installing Ninja Forms with no resolve.
