chore(error): improve error handling #28

rewrite handlers to introduce services
handlers now do only 2 things: Validate DTOs and Audit-Logging
services do business logic and all other error handling
remove additional from error type
500 errors do not contain internal errors
add missing pieces to spec
extend validator messages
