Also deal with ‘no incident’ situation > general bucket of ‘photos’
Through Updates
Upload form control > show button with text "Add Photo" instead of current browse button.
Make more intuitive: link on Home screen; so there will be 2 ways to add a photo

All options added. The text on the button is "Select", since the label already mentions that you need to select a photo.
