Fix harmony-api and HA MQTT switches

I have harmony-api working (can confirm the web API up on port 8282), but I haven't seen the switches in HA aren't picking up state changes.

I suspect it may be because I never upgraded harmony-api to 2.0 which supports multiple hubs.

This was 2 parts:

• update harmony-api's mqtt host to be localhost, since it's using net=host
• update mqtt switches to use new mqtt topics
