Update... just discovered that this issue happens when I have a 4 gb sd card
inserted
in the console. w/ 2 gb sd works fine

I've got a 4gb SD working fine with rev14. Can you try copying the /usb-loader/
folder and contents from your 2gb to your 4gb to see if that helps?

Re-installed cIOS36_rev10 and coverfloader 1.1 works like a charm with 4gb SD
card

Same deal here. I am using System Menu 4.1 and cIOS 38 rev 14 and the app
crashes
just before it gets to 'Initializing Disk' on the loading bar.

Followup to my previous, if I use a 2gb instead of an 8gb SD card, everything
works
normally.

Maybe a litle bit of topic but...

Some people seems having smal issues with some SD cards in general on the wii.

For me EX, I could't make an NAND backup on mine before I fully formated it with
Panasonic SDFormater v2.0 and didn't copy anything else on it. If there already
was
data on the sd-card the backup failed. Even the bootMii homebrew loader can't
recognize any of my second time copied (second time used in computer) data on
the
sd-card. Homebrew Channel and almost every other app/loader reads the new data
fine.

boricuahomeboy: You can try to format your 4gb card with the above SD-Card
software
(for me it didn't work with windows standard). And then copy your data back to
it and
then tell us how it went?

Are the people who say 4GB SD cards using SD cards, or SDHC? It does NOT work
on my
8GB SDHC. It does however work on my 4GB non-standard SD, and my 512MB SD.
It's a
pretty big inconvenience for me though because I had to pull the 4GB card from
another device that I'd rather use it in, but hopefully it gets fixed soon.

I'm using a 8GB Sandisk Ultra (class 6) system firmware 4.1, cIOS-rev14, and
Coverflow 1.1... Same error as everyone else, stack/code dump. Also the
curious
side effect of breaking cover art in Waninkoko's USB Loader 1.5.

As others have said, i tried the 1.0 loader on a 2GB card and everything works.
On
4GB card (Sandisk) i get a code dump. rev14 installed.

I also got the stack dump message using 4.1u with rev14 and with a 4gb SanDisk
Ultra2
SDHC card. I changed to a 1gb cheap card and it works.
