Subtotals should be on the same line as the subtotal heading. #25

Currently the subtotals are on the line after the subtotal title. The
subtotals should be on the same line as the "subtotal" text and it is up to
the user to properly format their report so subtotals don't end up on top
of the subtotal text. 

Some work for formatting can be done to give the first column enough space
to contain the subtotal text and the subtotals if the first column is being
subtotalled.

Revision 814 put the 'subtotal' text on the same line as the subtotals. 
Formatting
can still be done to properly space the subtotal text and the first column 
value if
desired.
