Image upload issue #10

I have uploaded this zip content in my org but i have an issue.......at my visualforce page.....after changing parent id to my account record in visualforce page code........when i am attaching a image then nothing happen .........progress bar stop at 0% no process after there.................need urgent help.............thanxzzzzzzzzzzz
