The PR finishes the PITR work for the operator.

Handle all extension reconciliation in the restored instances
Fence all restored instances and do not unfence until all extensions are installed (even on the primary)

As of now, adding my 1 test to this PR causes multiple integration tests to fail. I am researching why this happens to see if something can be done.
