What steps will reproduce the problem?
1. Install Alfresco
2. Start clean Alfresco and ensure it is working properly
3. Stop Alfresco
4. Restore alf_data folder
5. Start Alfresco and ensure it is working properly
6. Stop Alfresco
7. Place appropriate alfresco-pdf-toolkit-1.1.1.amp in /amps and
alfresco-share-pdf-toolkit-1.1.1.amp /amps_share folders.
8. Run /bin/apply_amp.sh script.
9. Start Alfresco
10. Find a .pdf document in the document library and click More...

What is the expected output? What do you see instead?
I expect to see the toolkit options (Merge, Split, etc.). I see the normal OOTB
Alfresco options.

What version of the product are you using? On what operating system?
Installer installed (Easy) Alfresco Community 4.2.d (3/28/13 nightly build),
Fedora 17 (Linux alfresco 3.8.4-102.fc17.x86_64 #1 SMP Sun Mar 24 13:09:09 UTC
2013 x86_64 x86_64 x86_64 GNU/Linux)

Please provide any additional information below.
I see the options when creating a rule. The installation via apply_amps.sh
provided no errors. If I check the alfresco.war and share.war files using
alfresco-mmt.jar, the amps are listed as installed:

I was reviewing the catalina.out and found the following:
 2013-04-09 19:52:45,755 WARN [repo.module.ModuleServiceImpl] [localhost-startStop-1] A previously-installed module 'org.alfresco.extension.pdftoolkit' (version 0.98.28) is not present in your distribution.

I haven't tried installing the amp into a clean alfresco install, since the
message is looking to upgrade from the previous version that was installed
another version of alfresco, maybe that's part of the reason it's not working?

Looking into this now. It is quite possibly related to the upgrade.

I did get a chance to try installing the amp into a clean alfresco 4.2.d
community install and saw the same results.

Same issue here. Tried on 4.2.c release and 4.2.d nightly.
