Upgraded module to support Terraform 0.12 #20

@lsc Thank you for the initial pull-request. I have reviewed the changes, run all the examples, and just released v2.0.0.

PS: Don't pay attention to outputs/variables in the documentation. It is not up-to-date because terraform-docs is not there yet with Terraform 0.12 support.
