Configuration of Code Coverage and Logging in phpunit.xml

The configuration of code coverage and logging in phpunit.xml has been changed to be less confusing. Here is an example of what this configuration looked like prior to PHPUnit 9.3:

<?xml version="1.0" encoding="UTF-8"?>
<phpunit xmlns:xsi="http://www.w3.org/2001/XMLSchema-instance"
         xsi:noNamespaceSchemaLocation="https://schema.phpunit.de/9.2/phpunit.xsd"
         cacheTokens="true"
         disableCodeCoverageIgnore="true"
         ignoreDeprecatedCodeUnitsFromCodeCoverage="true">
    <!-- ... -->
&lt;filter&gt;
    &lt;whitelist addUncoveredFilesFromWhitelist=&quot;true&quot;

</tr></table> ... (truncated)
