Refactor project to follow FIG standards, ignoring all PHP 5.4 items.

Originally brought up in #15.

Refactoring to follow PSR-0 will mean that the usage and installation of this project will have to change. It will no longer be as simple as copying two files into the root DokuWiki install directory.

While I'm all for simplicity, this refactor needs to happen in order to increase the import success rate.
