Implemented goal, crystal and boss selection #186

Added new goals for Fast Ganon and All Dungeons in ALTTP. Added options to set required crystals for Ganon's Tower and defeating Ganon. Added options to set required bosses in SM to enter Tourian.

This PR is based on #158, so closing that PR when this is merged.
