Fix sample request with first round of feedback #54

  1. First select individual contact
  2. Intelligently determine if individual contact is related to customer
  3. Fill-in the contacts details, rather than the customers
  4. Add a boolean check box if we are just going to send them a regular unit
  5. Add in per kilogram as standard number field (up to 2 decimal places)
  6. Add in per kilogram request if goods are coming from the warehouse (only enable field if they are)
  7. Add in a specific status per line (pending availability, requested, ready to send)
  8. Add in ability to print a packaging slip
