Address David's review #207

Closes #206. This specifies the URI Template for the proxy (and how parameters are encoded), as well as the URI Template for forwarded requests. I think it captures things correctly based on my reading of RFC6570.

Thanks @DavidSchinazi -- please have another look. The new text is simpler and moves the encoding requirements up front and on the Client.
