Missing variant classifications #11

These variant classifications are missing from snpEff_ANN_mutation_type_dictionary_file.txt:

initiator_codon_variant&splice_region_variant
non_coding_transcript_exon_variant
splice_region_variant&non_coding_transcript_exon_variant
start_retained_variant

I will make a pull request to add them in soon.
