rename SHAPE_SIZE macros #36

And all calls to them. Probably TABLE_SIZE?

I want to get this done before checking in shape.inc.csd... moving this to pre-alpha.

$ grep -r SHAPE_SIZE * 2>/dev/null  | wc -l
      85

Also should change flexadsr.xml to use GRAPH_SIZE instead since it's not being read as a table, only displayed.

Changed in the existing core, and in everything not yet checked in.

Double checking, will close if everything looks good.
