Together with PR #164 this should provide a more general and clean way for source<->score navigation both for workspace internal and external files. The hyperlink creation code is a bit more complex than before (because there may not be an IFile and the document for annotation location generation may have to be obtained otherwise).
I also added an index for the hyperlink text as there may be more than one locations on the same pdf file.
