Combine file/edit toolbars and allow buttons to be added/removed #17

I don't know about OS X, but on Windows and Linux, having multiple toolbars
with handles and also having separators inside those toolbars doesn't look
good at all.  We should having a single toolbar at the top, but we should
allow the users to add/remove buttons as they prefer (e.g., some may want a
button for toggling the terminal, while some may not).

This is low priority, and not essential to the 0.4 tag, but I think it's
what should be done nevertheless
