same problem here if you choose "custom" in "Format"

Same here. Custom format seems to work for the reminder, but not for the event 
itself.

Skip that last comment, it did only "work" for the reminder because I had set 
the action to only create an altert 
at the birthday itself.

I downloaded the source code, though, experimented a bit with it and found the 
source of the error:
There's that point in getAlertText where you are trying to replace %tel% by the 
contact's phone number. That's 
where something goes wrong – don't ask me what, I'm not really able to write 
an AppleScript myself, though I 
can more or less read them.
Anyway, I commented out the tell/end tell block where you try to get the phone 
number and replace %tel% by 
it, and then everything worked as expected.

Thx @ zottel for finding the failing line :-)
I fixed it by checking if phone info exists and taking the value of the first 
phone entry then.

Feel free to build it yourself by using my attached replacement 
main.applescript until Winnie finds some time to 
fix it.
