Application Detail: Family Information

As an admin, I would like to be able to see the family contact information for a particular application on the Application Details page.

Note: This ticket only references this area of the Application Details page.

Acceptance Criteria:

• [x] This information should be displayed below the Application Status bar, ideally on the right-hand of the screen
• [x] Should display the name of the Family Representative (first name & last name) from the application
• [x] Should display the Family Representative's email address
• [x] Should display the Family Representative's Phone number
• [x] Should display the Family Representative's relationship to the patient
• [x] Should display the Family Representative's request to be communicated to in Spanish (Espanol)

@bwalborn Please standardize the following font size and font color from:

@bwalborn The font size and color for the Family information is still off. Please follow the color and size formatting as the application overview section. The data from the database should be black, while the field names are a bit grayed out. See below:

@hmichele I updated the fonts in #120 #121 and #122 to reflect these changes but the branch there under (#120) hasn’t been merged yet. Once merged all the fonts and styles should be the same and fixed.
