HIPAA and Survey emails subject need additional information #365

Emails that may come back with attachments should have Patient last name, hospital city and state in the subject line, so the attachment can be saved to the relevant application folder.

Acceptance Criteria: [ ] When the HIPAA form request is sent, the subject needs to include the patient last name, and the hospital's city and state as it appears in the database. image.png
