PATIENCE-COUNTER isn't reset if you are there when the toll gnome falls asleep. Therefore, if he wakes up again he won't go back to sleep unless you leave the room. See I-GNOME.
Normally this isn't a problem because he gets annoyed and kills you the next time you wake him up. However, there are two ways to wake him up: Either you wake him, or you try to REZROV the gate while he's sleeping. He never kills you in the latter case, though he will do if he's awake when you do it.
To reproduce this, either REZROV the gate or wake the gnome. Wait for the gnome to fall asleep, then REZROV the gate. At this point, no amount of waiting (well, unless the counter wraps around at some point) will cause the gnome to go back to sleep again.
