Offset anchor links while taking the fixed header into consideration #417

This fixes the anchor link offset, which did not previously take the fixed header into consideration.

Is it possible the commit(s) got messed up? The changes in the current PR all seem unrelated.

@veksen Nah, I just threw in a few changes in the README, because it had a few issues.

If you insist, I can split the changes to the README as well as the changes directly related to this PR in two separate commits.
